Individualized Team-Based Care
To protect our patients and staff, all people entering a BCH Primary Care
facility will be required to wear a mask (exceptions include children
<2 yrs. old or medical reasons such as trouble breathing, unconsciousness,
or someone unable to remove a mask without assistance). *Bandanas, neck
gaiters, and masks with valves are not acceptable.
Thanks to our community's compliance with public health and safety
recommendations, we are able to reopen many of the services we offer at
BCH. It is safe to come to BCH for your health care needs.

Primary care serves as the first point of contact for your family’s
everyday health care needs. Our owned-and-operated primary care clinics
include both family medicine and internal medicine specialists dedicated
to providing ongoing and comprehensive health care for their patients.
BCH family medicine specialists provide medical care for all ages, often
treating multiple generations of the same family. Our internal medicine
specialists are uniquely trained to provide the full spectrum of care
for adult patients 18 years and older.
We offer a personalized, team-based approach to patient care, from diagnosing
and treating sudden illnesses to managing chronic conditions and providing
routine health screenings and vaccinations. In addition to physicians,
our integrated network includes care coordinators, educators and specialists
to ensure our patients always receive the best and most appropriate care.

Live Better Philosophy of Care
BCH’s twelve owned-and-operated primary care clinics are pleased
to offer Live Better, a more personal and coordinated approach to medical
care. Live Better is guided by the philosophy of a national health care
movement known as Patient-Centered Medical Home (PCMH). Our Live Better
clinics have received national recognition as certified patient-centered
medical homes from the National Committee for Quality Assurance (NCQA),
a private, non-profit organization dedicated to improving health care quality.
Live Better makes you the central focus of a dedicated Care Team led by
your primary care provider, working with you to meet your health needs.
