Getting a Copy of Your Medical Record
In order to obtain copies of your medical record, you must submit a
medical records release form.
Here are all the ways to obtain a copy of your Medical Record:

By Phone. Call us at 303-415-7760 Mon-Fri 8 a.m.-5 p.m. and request these forms to
be mailed to you.
2.
Visit In Person. Obtain a form in person by coming to the
Foothills Hospital (near Main Admissions)

Please note that there is a charge for personal copies of medical records.
This charge is determined by the Colorado Department of Public Health.
Making a Change to Your Medical Record
For your privacy, any updates or corrections to your medical record must
be requested in person with a photo ID.
Changes cannot be made over the phone.
