BCH will solicit comments on our 2020-2022 CHNA during an online public
meeting. You’ll have an opportunity to learn about BCH’s current
strategies for delivering innovative solutions that help improve the health
of community members and patients in five identified areas: Boulder Community
Health, like all Colorado health systems, uses a systematic process called
a Community Health Needs Assessment (CHNA) to identify and analyze local
health needs and assets in order to develop meaningful strategies to address
those needs.
- Chronic disease management and traumatic injury
- Mental health, including chronic pain management and substance abuse
- Wellness and preventative health, including aging of the population and
access to care
- Community education
- Care coordination of those enrolled in Health First Colorado (Colorado’s
Medicaid program)
The CHNA public meeting will be streamed on this web page on Wednesday,
April 14, from 6 to 7:30 p.m. Key leaders from BCH will review the major
actions taken to address these community concerns. Meeting participants
will be able to provide feedback both during and after this session. Please
