BCH Community Health Needs Assessment Public Meeting 2021

As our community’s independent nonprofit health care system, Boulder Community Health pursues a vision of creating and caring for the healthiest community in the nation. To achieve that goal, we perform an extensive health needs assessment every three years. We identify our community’s most pressing health care needs in a Community Health Needs Assessment (CHNA), and then identify the services, facilities and community education needed to address those concerns.
You’ll have an opportunity to learn about BCH’s current strategies for delivering innovative solutions that help improve the health of community members and patients in five identified areas:
- Chronic disease management and traumatic injury
- Mental health, including chronic pain management and substance abuse
- Wellness and preventative health, including aging of the population and access to care
- Community education
- Care coordination of those enrolled in Health First Colorado (Colorado’s Medicaid program)
