BCH Foundation's Heart to Heart fund provides financial support for cardiology patients

The Boulder Community Health Foundation's Heart to Heart fund has been helping patients afford cardiology care since February of 2023. This fund provides financial support for Boulder Heart cardiology patients in the prevention, treatment, and management of their cardiac health. When nominated by a care provider, patients become eligible to receive a grant to help support supplemental health needs.

Possible funding opportunities, determined by the care team at Boulder Heart, may include but are not limited to:

  • Support for cost of transportation to and from appointments
  • Support for co-pays, clinical, and medication costs
  • Support for the cost of medical equipment like blood pressure cuffs and cardiac monitors
  • Access to integrative care and wellness services like nutrition, fitness, massage, acupuncture, and reiki
  • Access to behavioral health care

The donors who established the program hope that Heart to Heart will continue to provide financial assistance to cardiac patients so they have one less worry and can focus on their health, wellness, and recovery. In 2025, over 200 Heart to Heart grants were approved for Boulder Heart patients totaling $135,730 in distributed financial aid.

Since 2024 alone, funds distributed have more than doubled, making the Heart to Heart fund the most utilized Patient Assistance Fund at the BCH Foundation. The most common diagnoses for those receiving Heart to Heart funding are Coronary Artery Disease, Atrial Fibrillation and Heart Failure. Funding has supported medication costs and transportation; integrative services like massage, Reiki, acupuncture and nutrition; as well as socio economic needs like rent or mortgage, groceries and child care costs.

"During a challenging time this past December and January, my husband faced serious health issues, which led to the unexpected need for a very costly potassium medication with a high co-pay. On a fixed income, this was a heavy burden we weren't prepared for. That's when we were incredibly fortunate to be offered the assistance of the Heart to Heart Fund. We are beyond grateful that this support exists. We can’t thank you enough for being there when we needed it most."
