Relocation Update
Our Adult Outpatient Rehabilitation program is moving to the Community Medical Center in Lafayette. The last day of treating patients at the current Broadway/Balsam location will be on Tuesday, April 9, 2019. All rehabilitation services resume at the new permanent home in Lafayette on Monday, April 15, 2019.
The move includes all programs, all services, all of your medical records and our entire interdisciplinary team of experienced and compassionate therapists. All of our physical therapists, occupational therapists and speech-language pathologists are transitioning to the new site.
