BCH Foundation Announces Nine 2026 Community Collaboration Grant Recipients

The Boulder Community Health (BCH) Foundation has granted over $440,600 in 2026 Community Collaboration funding to nine nonprofit organizations in Boulder County to address social and economic barriers to health for BCH patients.

The Community Collaboration Fund impact

In support of the BCH vision of partnering to create and care for the healthiest community in the nation, in 2018 the BCH Foundation established the Community Collaboration Fund. Since 2018, the BCH Foundation has been investing in Boulder-based community organizations that offer unique opportunities to help patients reach optimal health.

Because optimal health cannot be reached with medical care alone, this Fund aims to address the social and environmental needs of patients, in partnership with the organizations receiving this funding.

The BCH Foundation Grants Committee originally planned to award up to $350,000 for 2026. However, based on the strength of the applications for partnership and the potential impact, the Committee voted to award a total of $440,603 for community collaboration this year.

“These partnerships recognize that health doesn’t start or end in a hospital - it’s shaped by housing, food, connection, and access. This funding is about meeting people where they are and closing the gaps that medical care alone can’t,” says Grant Besser, President of the BCH Foundation and VP of Community Engagement for BCH.

Community Collaboration funding focus – bridging the gaps in care

The community collaboration funding is focused specifically on helping BCH patients who may be faced with social and economic difficulties in addition to medical conditions.

In the health care setting, these patients are referred to as “complex,” acknowledging that the many factors contributing to someone’s overall health are often compounding and difficult to organize and navigate – especially when funds are limited.

The BCH Foundation’s investment in these community partnerships aims to build bridges between a patient’s medical care and the organizations providing support outside the health care system.

Based on patient screening data collected in clinics and survey results from our Community Health Needs Assessment (CHNA), the Community Collaboration funding is focused on three priorities:

1.  Support for older adults with complex medical needs

2.  Access to health care support services for those who are uninsured or underinsured

3.  Timely mental health care options and care navigation

 

2026 Funding Recipients

Organizations selected in 2026 bring unique opportunities for patients to achieve optimal health despite social and economic challenges. Five recipients* selected this year have received grants in the past, demonstrating the depth of support growing between the BCH Foundation and its community partners.

Project Angel Heart* – Partnering to support patients with Medically Tailored Meal delivery post-discharge. Second year in a multi-year grant awarded in 2025.

Nine nonprofits have more money to use to address social and economic barriers to healthcare after Boulder Community Health Foundation awarded more than $440,000 in its annual Community Collaboration Fund grants.

In its ninth year, the grants aim to invest in local organizations that support patients outside of hospitals, according to a release. Grant money is meant to help organizations that support older adults with complex needs, expand uninsured or underinsured patient care or provide timely mental health care and navigation, the release states.
