Information on COVID-19 Vaccination for Children Age 12-17

As of 8/18/21, the BCH COVID Vaccine Clinic will be open on Tuesdays and Fridays from 9 a.m. - 6 p.m. by appointment. Walk-ins cannot be guaranteed a vaccine and may experience longer wait times.

You may schedule an appointment by calling 303-415-7777 or using the MyBCH portal.

On May 11, the U.S. Food and Drug Administration expanded use of the Pfizer vaccine for COVID-19 to include children age 12 to 15 years old. Please note: The only vaccine option approved for this age category is the Pfizer two-dose vaccine series. To read the expanded Emergency Use Authorization for the Pfizer vaccine, click here.

BCH is currently administering Pfizer vaccine to children age 12 to 17 at our COVID-19 Vaccine Clinic located at the Della Cava Medical Pavilion (4801 Riverbend Drive, 2nd Floor). The Vaccine Clinic is open Tuesdays only from 1 to 6 p.m.

Due to high demand, appointments are strongly encouraged. Students age 12 to 15 can schedule an appointment by calling 303-415-7777. Students age 16 and older can set up a MyBCH portal account to schedule an appointment at Walk-ins may experience long wait times or be turned away due to high volumes.

Requirements for Vaccination
Children age 12 to 17 do not need to have a parent or legal guardian present at the time of vaccination, but written or verbal consent is required.

To provide written consent, please complete the attached consent form and send it with your child to receive their vaccine. Verbal consent may be provided over the phone. To provide verbal consent, your child must be able to reach you by phone at the time of vaccine administration. If you cannot be reach by phone, please make sure you complete a consent form or your child will not be able to receive a vaccine.

Timing the COVID Vaccine with Other Vaccinations
The BCH infectious diseases team understands that your child may be due other vaccines at the same time you wish them to have the COVID-19 vaccine. The COVID-19 vaccine may be administered without regard to timing of your child's other vaccinations. This includes simultaneous administration of COVID-19 and other vaccines on the same day, as well as co-administration within 14 days.

Important Information

Boulder Community Health is following current guidelines from the Centers for Disease Control and Prevention (CDC) that call for patients and visitors to continue wearing masks at Foothills Hospital and all BCH patient-care facilities.
