County. Our clinic is conveniently located at the Community Medical Center
(CMC) in Lafayette:

To schedule an appointment, call 303-415-4340. Our operating hours are
Monday through Friday, 8:30 a.m. - 4:30 p.m. Provider hours are Tuesday
through Friday, 8:30 a.m. – 4:00 p.m.
If you are injured on the job:
- Always notify a supervisor immediately, or as soon as possible, so they
can authorize treatment. Once treatment has been authorized (or if your
supervisor is not immediately available), call 303-415-4340 for an appointment.
- For minor injuries during the hours of 8:30 a.m. - 4 p.m., Monday - Friday,
you may contact our office at 303-415-4340 and let us know if you are
on your way. Although we will work to accommodate walk-in patients, we
ask that, whenever possible, injured employees call for an appointment
at 303-415-4340 so that we can ensure the most appropriate and convenient care.
- For life threatening injuries, call 911 and ask to be taken to one of the
Boulder Community Health facilities.
