Boulder Valley Care Network nurse care coaches for St. Vrain Valley School District! They are winners of the 2018 Boulder
County Healthy Community Awards from the Boulder County Public Health
Department.
They were honored at a ceremony yesterday in Longmont.

The award is given to those making a difference by positively impacting
the health and well-being of members of our community.
As BVCN
nurse care coaches, Carla and Jerri work with employees of the St. Vrain Valley School District
to help keep them efficient, focused, and healthy. They help employees
find the motivation to achieve career and health goals, support individual
employee’s emotional and physical needs, and customize individual
health programs.
Kudos to the St. Vrain Valley School District, who offer their employees
access to nurse care coordinators like Carla and Jerri through membership in the
Boulder Valley Care Network, a group of physicians and providers striving to make health care in our
area more accessible and easier to navigate.
Employers who partner with BVCN can give their employees access to nurse
care coaches, along with a network of over 300 medical providers in the
Boulder area committed to a collaborative approach to coordinated care.
Boulder Community Health and St. Vrain Valley Schools are current members
of the BVCN,
with more area employer groups expected to join the BVCN in the coming months.
