As a division of Boulder Community Hospital, the Boulder Center for Sports Medicine strives to allow all members of the community to have access to the care it can provide. To that end we have contracts with most insurance plans.
Anthem; Blue Cross Blue Shield
Great West Life
HMO Colorado; Blue Cross Blue Shield
Kaiser PPO Plans
Pacificare (no Physical Therapy benefits for the HMO Plan)
Rocky Mountain Health Plans
United Health Care
HMO Plans must have William Burton, MD listed as their primary care physician in order to see Andy Pruitt, PA. We also work with most major auto plans.
Kaiser patients can only be seen in physical therapy if the patient has written authorization prior to scheduling with us. There are no Kaiser benefits for physician office visits, physican assistant office visits, or x-rays.
We Can Help
It is important for you to contact your insurance company so that you fully understand the coverage your policy offers.
Boulder Center for Sports Medicine is considered a Hospital Outpatient Facility. This is important to mention to your insurance company because these benefits can be different from a facility not owned by a hospital.
We know how difficult it can be to understand insurance rules and know what questions to ask. Don't hesitate to let our professional staff help you navigate the insurance maze! Our staff members can give you the right questions to ask your insurance company about your coverage.
Please note that some prices listed on this web site are cash pay prices only and insurance does not apply.
If you wish to use your insurance benefits for services such as gait analysis and bike fitting, medical billing charges apply. Please ask about this when scheduling.
Insurance benefits do not apply to any of our fitness classes, Sport Science performance services, or to sports massage (massage that is not a part of physical therapy).
Please call our insurance office if we can help. Kathy can be reached at (303) 441-0557.
