HMO Plans must have William Burton, MD listed as their primary care physician in order to see Andy Pruitt, PA. We also work with most major auto plans.

Kaiser patients can only be seen in physical therapy if the patient has written authorization prior to scheduling with us.  There are no Kaiser benefits for physician office visits, physican assistant office visits, or x-rays.

Boulder Center for Sports Medicine is considered a Hospital Outpatient Facility.  This is important to mention to your insurance company because these benefits can be different from a facility not owned by a hospital.

If you wish to use your insurance benefits for services such as gait analysis and bike fitting, medical billing charges apply.  Please ask about this when scheduling.

Insurance benefits do not apply to any of our fitness classes, Sport Science performance services, or to sports massage (massage that is not a part of physical therapy).
