In order to obtain copies of your medical records, you must submit a HIPAA compliant release form to the Medical Record Department. This may be done in different ways: 1. Phone the Medical Record Department at 303-440-2079 and request that a form be mailed to you. You may also come to the Medical Record Department and obtain a form. 2. CLICK HERE to print out a PDF version of the form. Then FAX or mail to: Medical Record Department Boulder Community Hospital PO Box 9019 Boulder, Colorado 80301-9019 FAX: 303-440-2359 Please note that there is a charge for personal copies of medical records. This charge is set by the Colorado Department of Public Health. The charge for your personal copies is 10 cents per page, plus postage.
