In order to obtain copies of your medical records, you must submit a HIPAA compliant release form to the Medical Record Department. This may be done in different ways: 1. Phone the Medical Record Department at
303-440-2079 and request that a form be mailed to you. You may also come to the Medical Record Department and obtain a form. 2. CLICK HERE to print out a PDF version of the form. Then FAX or mail to: Medical Record Department
