OHS Has Moved to Lafayette

You can still find our physicians at Foothills three half-days a week, but we are now primarily located in Lafayette

Boulder Community Hospital's Occupational Health Services (OHS) is the most comprehensive provider of occupational medicine in Boulder County.   The primary OHS clinic is conveniently located at the Community Medical Center (CMC) in Lafayette, 1000 W. South Boulder Rd., Suite 214, Monday-Friday.  We will continue seeing patients in Boulder on Monday and Wednesday mornings and Thursday. Please call for hours.  To schedule for either location, call (303) 604-4660.

Occupational Health Physical Therapy Services

Physical Therapy Services are provided by our well known and trusted OHS therapy staff, now employed by Boulder Center for Sports Medicine East in Lafayette.  There are also limited appointments available at BCSM's Boulder location.  Please call (303) 926-2665 to schedule therapy.

Designating OHS is easy!  Simply download the forms below, indicate the care choices that are right for your business, and either fax or e-mail them back to us.  Instructions are on each page.   Then, contact your Work Comp Insurance Provider and let them know that you have selected us as your Work Comp Care Provider.

Where do you go if you are injured on the job?

Always notify a supervisor immediately if possible.  For minor injuries during the hours of 8:30 am -4:00 pm, Monday - Friday, you may contact our office at (303) 604-4660, and let us know you are on your way.  Our convenient walk-in clinic is located in Lafayette at 1000 W. South Boulder Rd., Suite 214, Lafayette, CO.  Again, it is always best if you let us know you are coming.

For more serious injuries, call 911 and ask to be taken to the nearest Boulder Community Hospital Facility.

     * Drug testing must be completed within 24 hours after an injury and there must be a Drug Testing Contract on file signed by the Employer.
